Description : An examination was made of relevant research and literature in the area of Attention Deficit Hyperactivity Disorder (ADHD) and on theories leading to the development of current Diagnostic and Statistical Manual of Mental Disorders, Fourth Edition (DSM-IV) diagnostic criteria. The DSM-IV presently recognizes three subtypes: ADHD-Predominately Inattentive Type (ADHD-I), ADHD-Predominately Hyperactive-Impulsive Type (ADHD-H), and ADHD-Combined Type (ADHD-C). Although there has been much interest in the hyperactivity component of ADHD, the components of attention have generated less activity among researchers. A diagnosis of ADHD implies attentional deficits; it has not been validated, however, that the presence of attentional deficits necessarily equates with hyperactivity. Although ADHD-H involves hyperactivity and impulsivity, with problems of inattention, ADHD-I involves attentional deficits but not necessarily hyperactivity and impulsivity. Another fundamental difference between the two subtypes may also involve specific deficits of the attentional components. Shelton and Barkley (1994) suggest that the subtypes of ADHD may be etiologically different with ADHD-I having more deficits in the selective component of attention, and ADHD-H in the inhibition regulation of attention or sustained attention. Various instruments currently used to assess ADHD focus more on hyperactivity than on differentiating between the various dimensions of attention. It was proposed that if these two subtypes of ADHD have different neurological etiologies and subsequent differences in attentional components, then instruments that distinguish between the components of attention might improve a differential diagnosis. This study involved children ages 10-15, who were placed into one of three groups: Control, ADHD-I, or ADHD-H. The following instruments were used: a parent and child self-report questionnaire, Coding subtest of the WISC-III, Digit Cancellation Test, Trail Making Test: B, and the California Assessment Package (CalCAP). Criteria for subject selection included a minimum estimated intelligence of 80, two years continuous residency, and a 12-hour cessation of any stimulant medication. Statistics included analysis of variance with planned comparisons and a factorial analysis.
