Description : This volume focuses on the ethical and philosophical issues that arise in an aging society, and the implications of these issues for healthcare and social policy. After a brief overview of biomedicine's changing approach of ageing and longevity and of the new expectations that these changes generate, various ethical, social, and policy issues that surround aging and longevity are discussed. First, the images and social meanings of aging and old age in our society are explored, including their normative dimensions and implications for policy. Next, ethical issues in the care for frail elderly are discussed, as well as notion of good care and end-of-life decisions. Finally, the ethical and social implications of emerging possibilities for anti-aging and lifespan extension are considered. The book concludes with an overview of the relevance of the issues discussed for policy making on professional, national and international levels.

Description : In 1922, a coal miner strike spread across the United States, swallowing the heavily-unionized mining town of Herrin, Illinois. When the owner of the town's local mine hired non-union workers to break the strike, violent conflict broke out between the strikebreakers and unionized miners, who were all heavily armed. When strikebreakers surrendered and were promised safe passage home, the unionized miners began executing them before large, cheering crowds. This book tells the cruel truth behind the story that the coal industry tried to suppress and that Herrin wants to forget. A thorough account of the massacre and its aftermath, this book sets a heartland tragedy against the rise and decline of the coal industry.

Description : This book is an attempt to address the ethical issues raised by mental illness and its treatment by focusing on the question of autonomy. The mentally ill may be regarded as non-autonomous by virtue of irrationality, which may result in treatment models which deny them a voice. As a counter to this, some have moved to the other extreme and argued that the mentally ill must be regarded as fully autonomous in all circumstances, and consequently that all their wishes regarding treatment must be respected. This book examines the ethical consequences of such simplistic approaches towards autonomy and mental illness, and considers the ethical issues raised by specific forms of treatment. It is suggested in conclusion that improvement in the care and treatment of the mentally ill requires not only a fundamental change in social attitudes but also a less impoverished conception of autonomy than some of those currently employed.

Description : The pathophysiology of sepsis can be regarded as a series of steps, beginning with the invasion of normally sterile tissue by microbes and the elaboration of various pro-inflammatory mediators. The final common pathway is often the development of the multiple organ dysfunction syndrome (MODS). Whereas a great deal has been learned during the past quarter century about the inflammatory processes associated with sepsis (and other related conditions, such as ischemia/reperfusion injury), our understanding is far less developed with respect to the pathophysiological events that lead to organ dysfunction under these conditions. Nevertheless, efforts by both clinical and laboratory scientists are leading to new knowledge in this area. The chapters in this volume provide a state-of-the-art overview of many aspects of the pathophysiology of organ dysfunction in critical illness.

Description : The history of psychiatry is complex, reflecting diverse origins in mythology, cult beliefs, astrology, early medicine, law religion, philosophy, and politics. This complexity has generated considerable debate and an increasing outflow of historical scholarship, ranging from the enthusiastic meliorism of pre-World War II histories, to the iconoclastic revisionism of the 1960s, to more focused studies, such as the history of asylums and the validity and efficacy of Freudian theory. This volume, intended as a successor to the centennial history of American psychiatry published by the American Psychiatric Association in 1944, summarizes the significant events and processes of the half-century following World War II. Most of this history is written by clinicians who were central figures in it. In broad terms, the history of psychiatry after the war can be viewed as the story of a cycling sequence, shifting from a predominantly biological to a psychodynamic perspective and back again -- all presumably en route to an ultimate view that is truly integrated -- and interacting all the while with public perceptions, expectations, exasperations, and disappointments. In six sections, Drs. Roy Menninger and John Nemiah and their colleagues cover both the continuities and the dramatic changes of this period. The first four sections of the book are roughly chronological. The first section focuses on the war and its impact on psychiatry; the second reviews postwar growth of the field (psychoanalysis and psychotherapy, psychiatric education, and psychosomatic medicine); the third recounts the rise of scientific empiricism (biological psychiatry and nosology); and the fourth discusses public attitudes and perceptions of public mental health policy, deinstitutionalization, antipsychiatry, the consumer movement, and managed care. The fifth section examines the development of specialization and differentiation, exemplified by child and adolescent psychiatry, geriatric psychiatry, addiction psychiatry, and forensic psychiatry. The concluding section examines ethics, and women and minorities in psychiatry. Anyone interested in psychiatry will find this book a fascinating read.
