Reactive Arthritis Book Excerpt:

Reactive arthritis is a self limiting condition that is a member of the seronegative spondyloarthropathies, which can follow a recurrent and chronic course. It is also categorized as a oculomucocutaneous disease. Reactive arthritis most commonly affects young men with a triad of symptoms including arthritis, urethritis and conjunctivitis or uveitis. The exact cause of Reactive arthritis is unknown, but it is believed to be the result of an infection. Specifically, the infection is thought to be precipitated by either Gram negative caused by Shigella, Salmonella, Campylobacter, Yersinia, or nongonococcal urethritis caused by Chamydia tracomatis. An association exists between spondyloarthritis, Reactive arthritis, HLA-B27 and anterior uveitis. Eighty four percent of HLA-B27 positive patients with Acute Anterior Uveitis have other B27-associated disease, specifically Reactive arthritis, ankylosing spondylitis or psoriatic arthritis. About 20-30% of patients with Reiter's syndrome develop acute anterior uveitis. The bilateral conjunctivitis seen in that condition has an acute onset and may be is observed before or at the onset of arthritis. No specific treatment currently exists for Reactive arthritis. Most of Reactive arthritis patients show a recovery from their symptoms within a few weeks. Currently, anti inflammatory agents are the drugs of choice for therapy. Corticosteroids, Anti-TNF agents and physical therapy are also recommended.

The Microbiome in Rheumatic Diseases and Infection Book Excerpt:

This book discusses the role of the microbiome in rheumatic diseases and details its implications for patient treatment. Recently, with technological advances, there has been significant research into the microbiome. This has enabled us to more profoundly understand its role in our immune system maturation as well as the role played by microorganisms in autoimmunity and the deeply related rheumatic diseases. This book comprehensively explains the emerging microbiome research through the interrelationships of biomedical sciences, including: immunology, microbiology, bioinformatics, and, with special emphasis, the clinical aspect of rheumatology. It examines the interplay between infectious organisms and major autoimmune diseases, including rheumatoid arthritis, psoriatic arthritis, juvenile arthritis, systemic lupus erythematosus, and vasculitis, and explains how to apply that knowledge to diagnostic techniques and treatment decisions. The international team of expert authors provides insight into current therapies and future interventions specifically targeting the microbiota and explores the impact of our deeper understanding on enhancing personalized medicine. The Microbiome in Rheumatic Diseases and Infection is an essential resource for rheumatologists, pediatricians, internists, microbiologists, and critical care providers caring for children and adults with rheumatic diseases.

Inflammatory Arthritis in Clinical Practice Book Excerpt:

There has been a revolution in treatments for inflammatory arthritis. Mostly, this is due to the introduction of biologics, particularly TNF inhibitors. These have changed approach to treatment in two ways. First, they appear substantially more effective than conventional drugs. Second, they are far more expensive than traditional treatment approaches. One consequence of this therapeutic revolution is a need to better understand arthritis and to place the new biological treatments into the context of existing management approaches.

Axial Spondyloarthritis A Clinical Approach Book Excerpt:

Spondyloarthritis refers to a group of inflammatory diseases affecting the joints and enthuses. Ankylosing spondylitis is the most common of these disorders. Other disorders include psoriatic arthritis, reactive arthritis and enteropathic arthritis. Spondyloarthritis can cause tiredness, back pain, osteoporosis, psoriasis skin rash, pain or swelling in the hips, ankles, feet, knees, wrists, etc. The diagnosis of spondyloarthritis is based primarily on physical examination and medical history. An X-ray of the sacroiliac joints or an MRI may also be suggested. Pain and inflammation occurring in spondyloarthritis can be addressed with NSAIDs, while corticosteroids can be injected directly into a joint or membrane to reduce joint swelling. In cases where these drugs are insufficient to manage the symptoms, disease-modifying antirheumatic drugs may be prescribed to prevent joint damage and relieve symptoms. This book covers in detail the new frontiers in clinical management of spondyloarthritis. Different approaches, evaluations, methodologies and advanced studies on spondyloarthritis have been included herein. This book is a collective contribution of a renowned group of international experts on orthopedics and inflammatory diseases.

Ankylosing Spondylitis Book Excerpt:

Ankylosing Spondylitis (AS) and related diseases called spondyloarthropathies (SpA) are the second most common form of chronic inflammatory arthritis after Rheumatoid Arthritis (RA). Unfortunately, most patients with AS and SpA are either never diagnosed, or they are inadequately treated. The current mean delay in the diagnosis of AS ranges from 5 to 10 years, and the presenting clinical manifestations of these diseases are wide-ranging and heterogeneous, and thus a variety of health care professionals see AS and SpA patients, from primary care physicians to rheumatologists and other medical and surgical specialists. With the availability of newer, highly effective treatments and better methods of musculoskeletal imaging, such as the MRI, there is an urgent need to improve early diagnosis and reduce any inappropriate delay in obtaining effective treatment. Earlier diagnosis and treatment can mitigate the severity of the disease, and make a meaningful difference for patient care and quality of life. The proposed book is designed for specialists and generalists alike, and is intenteded to expedite accurate diagnosis and initiation of appropriate treatment in order to alleviate unnecessary suffering. Part of the new Oxford American Rheumatology Library series, this pocket-sized book provides practical, evidence-based information for clinicians to aid diagnosis, treatment, and follow-up care for patients with AS and SpA. The book provides pointers to early diagnosis, prognosis, clinical manifestations, socioeconomic aspects, and disease management. It also provides comprehensive coverage of current treatment options, including the latest biologics, as well as non-drug therapies.
