I have been induced to these details from the. Difficulty I have experienced in this as well as other ope rations, from the defect of minuteness in their descriptions. Those who are called on to their performance alone feel that no fact relating to them is superfluous; while others, who consult such descriptions from curiosity only, complain with justice of long descriptions. Besides the general reasons in favor of minuteness, there is one which is particularly applica ble to this case. Description : Excerpt from The Boston Medical and Surgical Journal, 1842, Vol. 26 Now, my dear Sir, permit me to say I have often pitied you when I have imagined you sitting surrounded by piles of illegible manuscripts pressing for insertion, in grave and solemn counsel with yourself whether to print the dull thing in hand and thus encourage nascent genius, let the doctor's ideas shoot and fill up two 01 three pages of the never-to-be-postponed weekly; while, at the same time, you run the risk of letting down the character of, our new-england Medical and Surgical Hebdomadal, of endorsing yourself a numbskull and a dolt, and, finally, of having some dozen of your more astute readers turn up their noses in a paroxysm of hypercriticism, and loading your goodly sized, fair-faced, handsome missal with the taunting epithets of insipid, flat, intolerable, wishy-washy, jejune, and so on, and so on. Don't conclude I have left out of the account the possibility of your losing a subscriber if you refuse to print it. Not at all. But, be not of fended if I say that if any of your contributors, after the unvarying and stereotyped indulgence and courtesy you have many years manifested in meeting the perplexities of conducting the only medical journal that has. Existed in the Northern States, should abandon you for not giving him ia sertion, he must have either an empty head or a perverse heart. We fear that Dr. Draper is justified in the observation Ire is said to have made, when the Review was shown him, That it was too bad that the editor of the North American had caused his book to be reviewed by a person so grossly ignorant of the mere elements of che mistry, as to assert the oils and resins consist of carbon and hydrogen only.' It is a well-known fact that the resins are oxides, and most of the oils contain oxygen. Should such an article appear in the London Foreign Quarterly, or in any of those journals which systematically abuse everything that comes from America, we could at once appreciate the motive. But after the highest scientific authorities in London, Paris, Edinburgh, Berlin, Geneva, had spoken of this work so well, and treated it with such consideration. The heal ing and recovery from the operation were very slow; and the winter being very cold, there was but little apparent improvement in his general appearance until warm weather was established. Since that time, he has improved rapidly. He has now the appearance otz good health - is cheer ful and happy - can walk miles with as much ease and elasticity as any one, and, with every prospect of good health and a life of usefulness, he is actively engaged in making arrangements to go into business. For some months after the Operation he had a weeping of prostatic fluid; but so soon as his general health improved, this trouble disappeared, and he has nothing of it at this time. Description : Excerpt from The Boston Medical and Surgical Journal, Vol. 12: Wednesday, February 11, 1835 After dissecting away the tumor, the kali was rubbed over the raw surface of the base. The adjacent sound parts being carefully protected with dossils of lint, the kali was repeatedly applied to the surface, which was again and again cleared by rubbing of}' the dead portions after each application of the kali. This process, which occupied a considerable time, was daily repeated. Finally, the bone was laid completely bare, and in a few weeks a bone exfoliated, consisting of a thin lamella of considerable breadth. The vacuity which remained, and which was large, from three of the teeth having been removed, rapidly granulated and cicatrized, arid when I saw this patient a few years afterwards, I was astonished to find that the contiguous teeth had coalesced, filling up the deficiency in the jaw in a most extraordinary manner. Case II. - A gentleman, forty-five years of age, had a tumor of the upper maxilla, which completely filled up the space between the four incisors and lip, and was so large as to elevate the lip and deform the countenance. It was of a dark purple color, had a firm fleshy feel, and adhered immoveably by a broad base to the subjacent bone. I dissected back the lip, to expose the whole tumor, and then removed all that portion which the knife could reach, and afterwards freely rubbed the kali purum on the raw surface. The application of the kali was repeated every day for some lime, and subsequently every other day. Ultimately the alveolar processes were completely exposed, and exfoliating portions of the thin external laminae daily separated, so that in a few weeks the whole dead bone had exfoliated, and the surface which it exposed was afterwards speedily covered with granulations, which cicatrized, and left little deformity. Remarks. - These cases will suffice to show the advantages to be derived from this mode of treatment, for it is evident that by the knife alone it is impossible to destroy such tumors, as they grow in situations where it is often not practicable to saw off, or otherwise remove, the diseased portions of the bone from which they grow. Nor can the actual cautery answer the intended purpose, unless by such frequent repetitions of a painful operation as few would allow. The common lunar caustic is quite useless in such cases; as the length of time occupied by the formation and separation of an eschar renders it too inert a remedy, the growth of the tumor being more rapid than the destruction effected by the caustic; whereas the kali purum possesses all the advantages of the actual cautery in the rapidity of its action, and of the lunar caustic in the nicety of its application. The kali destroys the life even of skin, almost as quickly as the cautery, so that slough after slough may be produced, and a large portion of the diseased growth thus daily destroyed. Besides, it has the advantage of being applied with great precision to any particular part, by which only the diseased portion is destroyed, and the necessary quantity of bone denuded.
